Our office accepts referred and self-referred patients.
To make an appointment, call the office during business hours 765-966-1600 or 1-800-891-1633. Your schedule and the physician’s schedule are considered while making your appointment with our office.
We will need a complete address, your insurance information and a telephone number where we can reach you in case your doctor has an emergency and cannot see you at your scheduled appointment time.
If you are unable to keep your appointment, we ask that you kindly give 24 hours notice. Please contact the office if you are going to be late to an appointment – we may need to reschedule you accordingly.
We try to serve you as quickly as possible. Please arrive promptly for your office visit as this will eliminate delays and reduce waiting times. If you are a new patient, we ask that you arrive 15 minutes early so we can register you into our computer system. When possible, please bring the registration form that we sent to you or you may print and fill out our registration form from our Web site.
